School-linked and Accessible to the Community

The CVCC operates five school-based Family Resource Centers (FRCs), with each one providing a gateway to a full range of family and youth-centered services. Services are available to all members of the community, regardless of their circumstances. A call or visit to one of the FRCs is all that is needed is to connect to the resources and opportunities that exist in your community.

Making a Referral (CVESD and SUHSD)

To make a referral, click on the link below and completely fill out the form. Fax to (619) 427-6954 or email to info@chulavistacc.org . Please be sure to let parent(s) know to expect a call from CVCC regarding this referral.

Having trouble making a referral?

If you have difficulties either downloading or sending the referral, please email Brenda.Mendez@cvesd.org or call (619) 427-2119. You should receive confirmation of receipt within 1-2 school days. If you do not receive confirmation, please contact us.
